Email notifications no longer working

About a week ago our email notifications stopped working.  The form in question is http://www.jotformpro.com/form/41619341308956

Hi Peter,

I checked the Email History of your form and I can see that the Notification where sent successfully. You can also checked your form's Email History on your end. You can check this article for more information: How to view all your form Email History. I noticed that the sender email of your Notification email is indooroopillyswimming@gmail.com. Can you try to use noreply@jotform.com and test if the Notification will now work?
