I checked the other forums and found the answers to people with this problem before. I have even whitelisted JotForm on our OutLook account. However, the two people who are supposed to receive email notifications have not. What I am missing?

Since several of your notifications failed previously, this resulted in your addresses being placed on our bounce list. I was able to remove "firstname.lastname@example.org", however, "email@example.com" was placed on our providers suppression list. This needs to removed by our admins. I will forward the request to them so that it can be removed. For now, "firstname.lastname@example.org" should be able to accept the notifications once again if you have indeed whitelisted our IP's and domains.
We will update you here in this thread as soon as your address has been removed from the suppression list.

It appears that the email address in issue is no longer in the suppressed list. I have made two test email alerts to your email address and it shows to have been properly sent from our end. Can you please confirm that you have received to email alerts?

Thanks for looking into the issue. I looked at Patrick's email and he received both test emails. We even had one form come through from the website. Debbie received the same form. So everything is working great.
