    Hello,

This organization was signed up with Jotforms a while ago, but the person who did it has left and I have taken her position.  We are serious about using the service this year for our camp registrations and I am trying to see if I can create what we need before I purchase a service, but when I try to preview any of our forms it says they are disabled  What can I do?

I took a look and I saw your form "[Imported Form] Dropbox - 2015 Health Form.docx" which is against our terms of use (relevant excerpt):

Phishing You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately

As such I have immediately removed it from your account, but please note that if you create the same / similar form, your account will be suspended (as per our terms of use).

I presume that this has happened by accident, which is why I have mentioned the above so that you can create other forms, but keep in mind what would be against our terms.
