Not Receiving All Fields in MailChimp except Email

I've integrated my jot form to my mail chimp account and also embedded it onto our FB platform but when the contact information feeds into my mail chimp list, it only records the email address and not the first name, last name or phone number even though they are required fields. Can you help me with this?

Based on your above issue it seems you are having a problem with getting some of your form data to arrive in your mailchimp account through the integration.

I believe that most likely it isn't mapped correctly which I was able to confirm when I went into your form to investigate the problem.

You only currently have the "email" field mapped for MC.

To get the rest of the information you require and have it sent there you need to add and set up the mapping for the rest of your fields.
