Link not working

I created a form the other day and had two submissions but now today, it says my link is disabled. However, in settings, it is set to enabled. How do I fix this?

I'm checking this form link http://www.jotform.us/form/52523750141143 and it seems like you have deleted it, you can find it in your Trash folder.

I also see that your account was autosuspended, this usually happens when our anti-Phishing system finds a violation of our Terms of use in your form, I checked your live forms (Clone of Whitening Toothpaste Order Form and Whitening Toothpaste Order Form) and they don't seem to have any fields that violates our Terms, but please check our "Terms of Use" page, especially under the "Phishing" section to learn more about what are not allowed. I'm unable to check those forms that are in the "Trash" folder, if they are forms that are asking for sensitive information like credit card details, bank information or log in credentials, please delete them permanently by clicking the "Purge" button.
