Form not mobile responsive on iPhone 6?

Also i have big issues to see it nicely on mobile iam using an iPhone 6

I see you add the mobile responsive widget, aside from enabling the options in the Preferences and in the Form Designer Tool. Could you try removing the Mobile responsive widget instead? 

Which means we will only enable this option

I cloned your form for testing purposes,you can view my cloned form using this link: http://form.jotformpro.com/form/52525130253949. See if that will give you a better layout in your iPhone 6. I checked it in an emulator and this is how it looks like:

Thanks its working now but
i realy have a hard time to get the date picker to work on my mobile
When i try to pick a date it wont choose it and close the box ?
could you please assis on that matter , ---

I also noticed that Thursday, Friday and Saturday dates are unavailable to select, is this correct?
