The third issue is that when the form is viewed in a Chrome or IE desktop browser (or in the form builder itself) the drop-downs are missing with sad-face page icons in their place.

The form functions fine in a mobile browser.
This is probably a temporary issue to your end. Is it still showing the same error? I was able to view it here using IE 10 on Windows 7 and IE11 on Windows 10. What's your IE version by the way?
For making the changes, we recommend Google Chrome or Mozilla Firefox. Outdated versions aren't supported so we recommend using the new ones.
Also, try running the configuration again:
We are using Chrome Version 45.0.2454.85 m as our primary browser to build and test all Jotforms on our Windows 7 system.
When the drop-downs failed to load we tried IE Version 11.0.9600.18015 and encountered the same issue.
The issue persists even after we run the configuration again on the widgets.
This problem is secondary (the form is accessed primarily via mobile devices) and the name-selection issue is our primary concern for our deadline.

I've tested the form in several browsers version and I am able to see the form perfectly.

Is this embedded in a webpage? We need to isolate this issue so we expect more details about this.

You are correct - the webpage does appear to load correctly today in the previously troublesome browsers (Chrome Version 45.0.2454.85 and IE Version 11.0.9600.18015) on our Windows 7 system.
We were opening the URL directly - no embedding or anything, though we do plan to place it in an iframe in our app environment in the weeks ahead.
It appears to have been a temporary issue, (or whatever action you took on Sept 11 corrected it).
