I cloned your form to test it and it looks like the full information from your notifications does come through:
If there is information still missing from your, try creating a new notification:
http://www.jotform.com/help/25-Setting-Up-Email-Notifications
This will give you a fresh notification with updated field ID's.
If that still does not work for you, let us know and we will be happy to take another look.
