My form did not total correctly for 2 customers. How do I fix this?

I had 2 customers whose order totaled as zero.  I don't know how to fix it.

The issue is because of the form field "Order Package 1 - $40.00" you have in your form. You have set the "Calculation Value" of this check box field but only for one item. If you select any other item other than the first one "Pose A", the value will not be added in the form calculation widget.

I would suggest you to add the "Calculation Values" for all the items of your check box question and that should solve your problem.

I have tried to change this, but now it is totalling each pose as $40.00.  

My intent is that the customer chooses up to 5 different choices (poses) for $40.00. That is why I called it a "package" deal.

Do I need to make a different type of question?

Okay I have understood your requirement now. Yes it will be a challenge to achieve your requirement with "calculation Value" settings. There is an easier way to achieve your requirement. Here are the steps you should follow:

#1. Add a hidden text box in form and add a condition to update its value if any of the options of package 1 is selected. 

#2. Repeat the step #1 for package 2 as well.

#3. Update the "Order Sub-total" form calculation widget to read the value of both the hidden fields.
