Unable to submit the form. Hangs at Please Wait...

I filled out my form to test it and its says "Please wait..." and does not show a success page.  I didn't get the form results either.  What is wrong with my form?

Also can I get the form to submit to me after the first page (ie their name and contact info) in case they don't do the entire form?

I see that the spacer widgets on your JotForm is causing the issue. However, I've refreshed the widgets now on your form and I was able to submit the form. Please check and let us know if the issue still persists. We will be happy to take a look again.
