There is still an issue; emails are not being sent.

The problem I see with your email notifications is that you have not setup the notification accordingly, first , you must add at least one recipient, most of the notifications are empty

 

Secondly, rather than putting the multiple recipients in the conditional logic, you should just add them in the email notifier Recipient Email field.
