How can I have my form to make multiple selections in a category?

As a qualifier to join the VFW, a potential member must have served in a conflict, went to Korea for 30+ days, or received hazardous duty pay.  I have made a form where it would be nice if potential members can choose "multiple" qualifiers and on my form they cant.  Can you help?

I will also have this problem for selecting combat medals.

Instead of the Radio Button field which is for single selection only, use our Check Box field instead which will allow you to select multiple options:
