    UGH! I now somehow removed the red title that was on the form. I just wanted to add the jpeg on top. I still don’t know how to get the form back the way I had it. Please help!!!

    The best way to repair some issue if the undo option is not available for you (for example if you got out of the form) is to use the Form Revisions history option.

    It will allow you to restore back the working form even after few different edits were made and is a very handy tool to have.

    You can see more about it here: How to View Form Revision History

    Please do note that once you go back to some version, you will not be able to go in the version that is newer than that, so my suggestion is to always go back to the latest working form (you will see the form displayed as well as the edits log on the right.

    Ok, I did all what you said and I'm back  in good shape, thank you!

    I just need to make the three Quality of Food, Accommodations of Diets and Efficiency and Friendliness a little larger and bolder. Can't see where to do that? For font choices? bold, color, etc.

    I love the spacing of the questions toward the bottom of the form. Can we do that to the top? There is so much room up there.

    Lastly, I am trying to add a jpeg OVER the the star plate, but keep the red banner. Can I do that and how? I was able to add it at one point, but I lost the red banner :(

    I so appreciate your help. I have folks waiting on me for this form, yikes! Thank you Thank you!!!

    I need that image at the top that says POPCS to go over the plate! See screenshot...

    Also, do you see how the banner is longer than the page on the right? How do I fix that?

    I promise I have been trying to find all of this stuff on the forum site!

    There is a lot of custom CSS injected in your form, which causes the overlapping issues, so I went ahead and removed it all in my cloned version:

    Which now displays correctly: http://form.jotformpro.com/form/52578026479971?

    So, you can clone my form if you want to: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL

    To edit your form(fonts, colors, spacing etc), here is a tutorial to accomplish it with our Designer too: https://www.jotform.com/help/300-Form-Designer-Tutorial-Let-s-create-fantastic-forms-

    Ok

    Can you make it back to this? This is actually what I was trying to do! but have the double red lines from the plate around it if it works. If not, this is fine!

    Also, I'm sorry, but the background of the form was white. I think I know how to do that though.

    And is there anyway to make the form shorter, so less line space bc it's printing on an 8.5 x 11 sheet of paper. The top of the form does not have to be that big either, meaning the title takes up a lot of space, but don't need all that. I really appreciate all of your help. This should be the last part!!!

    And I'm really sorry, but the last link you sent does not come over right on my end :(

    Not even in preview.

    So no luck from anyone there? I have to tell you, this has been a total nightmare. I had one guy do all this stuff and drop off the face of the earth. Another guy comes in to fix it and does nothing.

    I have to submit this form tonight that I have been working on for days and it's a total wreck.

    I need help and nobody will respond. I don't understand that.

    Hi,

    Sorry for the inconvenience. I have cloned your form and it look like this.

    I didn't see any red title. Have you deleted it?

    I injected a custom CSS code to the cloned form and now it looks like:

    The injected CSS codes are below.

    #id_83 .form-image {

        top: 165px;

    }

    Do let us know if you need further assistance.

    Regards.
