Forms Keep Disappearing

I'm using these on a template-based web platform (FMG for financial advisors). I'm sure it's their issue, but I'm not getting support. When I embed the script code, the form may or may not show up, and if it does it might disappear later. Code gets completely erased from HTML. Any thoughts?

If the codes get stripped out, I think the platform do not support scripts. In this case, try using our iframe method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. You can find a script in the iframe embed codes, but if the web builder will strip it, the form should still work.
