The problem is that you are actually using two forms on your website - and each form must have its own submit button in order for the form to be submitted. Your current form on JotForm is this one:

    http://www.jotform.us/form/52575971722160

It only has a File Upload field on it, and a Mobile Responsive widget, without a Submit button

In order for your form to be able to accept submissions, it must have a Submit button that your users will click on. For this reason, I recommend that you create your entire form with the Full Name, Email address, File Upload, and other fields that you are trying to collect in your other form directly in your JotForm form instead.

You can use our easy fields from the Quick Tools section to add the name and email fields to your form.

I hope this helps. Please let us know if you need further assistance.

Thank you so much for your response. I added a submit button and it works.

However, it seems like I'm not having any luck remaking the form I made on SquareSpace. For some reason the text boxes aren't allowing me to write in them. Any help?
