Hello, and welcome to JotForm. :)

You can simply clone that form into your account to get you started.

Form already has a File Upload field on it to support file uploads, so all you will need to set up is your Email Notification. Please refer to the following guides for ensuring that your email notification is correctly set up, for you to receive email alerts of submissions:

    Setting Up Email Notifications

How to correctly setup email alerts to prevent email bouncing related issues

You can also make any other changes to your form that you need, such as adding other fields, or removing some of the predefined fields that you do not need.

The cost will depend on how many submissions you expect to receive each month, and how much storage space you will need. We don't limit any features on any of our pricing plans, so regardless of your plan - you will have access to all our features.

The free plan can receive up to 100 submissions per month, or 10 payment submissions. It can also receive up to 100 MB of uploaded files in total, though you can delete your submissions with uploaded files at any time to reclaim some of the upload space.

Our paid plans start at $19 / month for the Bronze monthly plan, and it supports up to 1000 submissions and 1000 payment submissions each month, as well as 10 GB of upload space.

You can see more details on our pricing page: http://www.jotform.com/pricing

Please let us know if you need further clarifications or assistance, and we will be happy to help.
