I use WordPress fro my website.
I made a form on JotForm and successfully added it to my site, via instruction from the JotForm site.
It worked perfectly for a couple months....now the fields aren't fillable.

I was able to see the problem on your jotform http://www.jotform.us/form/51186520659156
Some fields are not fillable because they were set as Read-Only.
To fix this, select the field and then view its Properties option.
You can then set the Read-Only option to NO.
Save the form and check/test again.
Let us know if this did not resolve the issue.

This fixed the issue. I must have been in there messing with things and didn't realized what I did.
