I want to have the headings and subheadings in the forms show when I receive the e-mail notifications. How do I do this?

    We are designing subscription forms to sell a digital magazine.  I am using agents in different regions to sell the subscriptions for me but we need a way to ID the agent so that I can pay them commission at the end of a month.  We therefore need an agent name/code to show when we get the JotForm in our e-mails.

    It appears you have asked two questions in you thread, I will address them sequentially.

    1. To include headings in your email you will have to manually design them withing the template since the form values are the ones that are added automatically from the form. The notification email editor has extensive HTML capabilities that you can use to design the headings and the sub headings.

    2. Regarding the identification of the agent in the form, if you have a finite number of agents and it is one agent per region you can add them to a hidden field using conditions  based on the regions selected (from a drop-down).
