I have customers telling me that they have submitted and we have no record.

We are truly sorry about this. This problem is fixed now.
The emails that were missing should arrive later on.
If for some reason the emails still do not come, I would recommend to double check that no fields are missing or set to "Please select" in the email notification settings.
And check if the email address is not in our bounce list:
