If you can provide us link of your form we can check on which account that form is created.
You can login to your account with email address or with username. If you have forgot your password you can reset your password:
1. Go to jotform login page
2. Click on the login button on top
3. Click the "Forgot Password?" button
4. Provide your username or email, and click on send reset button.
I have also checked your email address and I found quest account with two forms connected to your email address. Most likely you did not sign up and you created your form on guest account. If that is the case I would suggest you to sign up and create free account and then we can transfer forms from your guest account to your new registered account. Please use the next link to sign up:
