Hi, whenever someone tries to upload images, the form clears all the data which is very frustrating!

Hi,
I've really tried hard to encounter same error, but I didn't. I tried on different browsers, on mobile platforms, on macos and the pc.
Also, here is the submission from that form.
At that point, I suggest you to clone your form and try again.

Alternatively, you can clear your browsers' cache.
