The reason for that is the form: "My Encrypted Form"
You can see more about this here: http://www.jotform.com/terms/
I am citing the segment of interest:
Phishing You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately
Please do note that you would need to either:
1. remove the form completely, or
2. remove the fields that are asking for sensitive details.
Once you do your account should be enabled once again automatically, if not, do let us know and we would be happy to remove all the parts of the form that are causing the issue and enable your account.
Thanks for the response. I thought the encrypted forms would be a great way to receive secure information from our clients, but I guess not.
I've removed the offending form. How long can I expect it to take for the account to be reactivated?
Unfortunately the encrypted forms have been seen as if they can be used to capture various info, but this has not been so. The encryption forms are there only to allow you to capture the same data that you can capture usually, but with more confidentiality.
If you want you can take a look at this guide that answers most if not all of the questions regarding the encrypted forms: What are Encrypted Forms and how to use them as expert?
Now in regards to the forms being down, usually the account is enabled right away. It has still been disabled, but I have now enabled it for you so your forms are not active as well as your account.
