Having issues with the form loading correctly/consistently on my website.

    I embedded the form with code injection.  It works ok about 1/3 of the time.  Some times it does not load, other times it loads but the first selection box does not perform the correct action when the users select their option.

    Please be more specific? What is exactly not loading or working properly? I just tested your form, with the direct link and in the link to the webpage where the form is embedded, in both works correctly the conditions to Show/Hide depending on the selection of the first Drop Down:

    In what browser version, operating system, or mobile device have you seen the glitch with the form?

    I am using safari on mac os x, most current versions.  My customers are from all over the world on every platform combination you can think of.  

    The form works fine sometimes, but on my computer I have to refresh the webpage about every third time I click in.  Sometimes I have to refresh twice.

    I only have about 14 submissions and I am already getting emails from visitor about the form not loading.

    Direct visit's usually load correctly, but click in's from remote links are problematic

    It's quite odd, couldn't replicate the issue, I would recommend you to get a  Iframe code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code, when using Iframe embed method, pretty much nothing can break your form.

    Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the iFrame part in your webpage, example:
