How can we show additional questions if the user selects Yes?

    I have a form where individuals must answer a question with yes and no options. I want to add a contact information section that opens up if yes is selected.

    Question: Have you secured a practicum site?

    Answer: Yes

    Need to add only if yes is selected: Site Name, Contact Personnel Name, Personnel Email

    You can use Smart Forms using Condition Logic in JotForm.


    For your jotform    you can apply the method described on this user guide

    The condition logic I used was like this

    I am requesting assistance again on
    Under Rehabilitation Counseling click here:
    After individuals click “I approve” to answer the question “By approving this statement you accept that the comprehensive exam content will cover the material in the above listed course in progress and you are accepting responsibility for such”.
    I would like to have the page skip to the end of the form where is states “I confirm that I have read the above information. I intend to take the masters comprehensive examination on:” and continue to list all examination information including the submit button.
    I have went to conditions but it doesn’t seem to be working.

    The "Skip to page or end of form" condition only works with page breaks:

    This will redirect to certain pages within the form based on selections made.  This would be used if you have a very long form where certain pages do not need to be filled out by everyone.  It will not cause the form to scroll from one form collapse to another.

    If you have 3 page breaks, your form would be separated to 4 pages.  If based on a selection in page one, they would not have to fill out page two, you could setup the condition to skip to page 3 instead of going to page 2.
