I checked the URL where you have embedded this form - http://www.yourlawyer.com/topics/overview/request-call-back and I was able to see what you are reporting. Can you try to re-embed the form using the iFrame embed method and see if it will now work? To get the iFrame embed code of your form, you can check this article: Getting the Form iFrame Code.
