Why My Form is Disabled?

    I have created two forms in my account. Both were working fine till yesterday. But suddenly both the forms are disabled today.In my preference both are enabled and i have made only one submission on it. I created for educational purpose and its very urgent. please help me out

    The form called "TRAFFIC ACCIDENT REPORT" is a login form and on JotForm, it is not allowed to use login forms for security purposes. That's why, your account is disabled.

    I've deleted the form and activate your account. You should be fine from now on; but if you create another login form, unfortunately, your account is going to disabled again.
