Multiple signatures on a form and Notifications

    I need to find out how to get a "Supervisor" to fill out the form with their signature, then send it to their employee(s) who will need to sign the same form and add their own Staff employee notes. However, no changes should be made to the form previously filled out by the Supervisor. Also this will be sent to multiple people across our company. I would great appreciate your help.
