Autosuspended: Asking for credit card information

    I created a new form "Family Focus Travel Credit card authorization".  When I go to 'Preview' or when I try to embed the form on my website, there is a big box that says "This form is disabled".  Please "Enable" my form.

    Your account status is autosuspended. Your form is asking for credit card information. It is not allowed in JotForm. This is a violation of our terms of use.

    You need to use our payment integrations in order to charge the customer via credit card.

    In order to re-activate your account, we need to delete those forms. Let us know if you like to proceed. Thank you.

    Is this a new policy?  I have been using a similar form for at least 2 years.  Why am I suddenly not able to use a form to gather credit card information?  I do NOT need to actually process credit card payments.  I just need a form to collect credit card information.

    Hi,

    Please check our "Terms of Use", under the Phishing section. We do not allow forms that ask for sensitive information like credit card details, bank information, log in credentials, or the likes, they are considered phishing forms. We have an anti-Phishing system and manual reviewers that checks thousands of forms everyday.

    If you need to process payments, you will need to use our payment integrations as mentioned by my colleague. If you do not need to immediately or automatically charge the client or customer, you can set the manual authorization by following this guide: http://www.jotform.com/help/292-How-to-Enable-Payment-Authorization.

    Please delete the forms asking for credit card information and reactivate my account.

    I deleted the two credit card authorization forms and reactivated your other two forms.  You should be able to access your "Request a Quote" form once again.  My apologies for any frustration this may have caused you.
