I am receiving for submissions to my email. However I forgot my username and password. So when I try to reset my accout by creating new password I am not receiving the email! Help

Since you have mentioned that you are receiving emails on your email address when your form is submitted, I would like for you to confirm if this is the form you're referring to, http://www.jotform.us/form/52655194369163? If not, kindly give us the form so we can check it on our end. But, if that is the form, I would suggest you sign-up for an account where you will have a username and a password using your email address. The form was created and saved on a guest account(not subscribed). If you will create an account using the same email address, you will still have access to the forms created when your email address is still a guest account.
