Retrieving deleted form

Unfortunately Nadia, once deleted, it is gone forever. I did try to find it, but based on the result, it is long gone.

I did check your Trash folder as well, but even though it has a lot of forms in it, the form above is not one of them.

If you have embedded this for using source code embed option, then there is a possibility of creating a new one based on that, but other than that, there is nothing that we can do to restore it for you.
