I am a principal at a middle school and love this tool for doing teacher evaluation forms but I want the unchecked items to still show up to give the teacher feedback about what they might have missed in their lesson. Is this possible?

So far if I understand you correctly it seems to me that you want your form checkboxes to show up on your report or pdf even if they are unchecked? Is that right? Or?
Are you referring to the checkboxes themselves or just the options that weren't selected and you wanted those to show instead?
What you could do is create a fillable pdf as an alternative since this allows you to use checkboxes inside it whereas the Reports are meant for answers instead.
However, please confirm or explain more if I have misunderstood you.

Unfortunately, it is not possible to include include unchecked options in your form. You could create a single checkbox field for each option, then include empty fields in your submission data. However, only the selected options from a checkbox field will show if an option is selected. The unselected options cannot be forced to show.
For example, if you have 7 options in a checkbox field, if one is selected, only that will be forwarded with your submission data. If you have 1 option and it is not selected, the field label can be included in the submissions data, the option for the checkbox will not.
