This is the Jotform #10325828204 that appears on my source code on the website:
It shows I have 180 some submissions, but the report is blank, the edit field is blank, and customer has commented that no one has followed up on the contact form, because I don't believe they submissions are being executed...Yet, the form still shows up on my form list and appears to be active. What's up??

Your form with the ID 10325828204 is named "L2BH - Contact Form FRANCHISE SITE", you seem to be looking at the wrong form in the screenshot. This form has 519 submissions.
I can see that this form is receiving submissions regularly, and last 2 submissions were received today:
Your form email history also shows that notification emails have been sent to you when submissions were received on this form.
The form that you are showing in your screenshot has an ID 10881744157, and is named "L2BH Corp - Contact Form". This other form is currently empty, because it seems to have been saved without any fields on it. Last modifications on it was on June 08.
One possible cause of this could be that you were using a source code version of your form and embedded it in your website as such, while (likely accidentally) removing all fields from your form on JotForm. That way, your source code form would still attempt to submit to our servers, but our servers would receive blank submissions - other than IP, date and submission ID that is generated by our servers.
If you have a link to the page where this blank form, 10881744157, is embedded we could take a look to see if this is indeed the case. However, the form on the page you have linked to is your other form named L2BH - Contact Form FRANCHISE SITE.

Yes, indeed! This has been a continual source of confusion for me, as the forms were not named by me, and I do not have reason to fuss with them often enough to remember which form is called what. Guess it's time to take ownership and rename them so that I can stop confusing the two.

Unfortunately, that form was since edited on JotForm, and all form fields have been removed from the online form. In order for any form to submit data to our servers, form fields must match the ones on the online form in JotForm.
So we have a situation where the obsolete source code of the form that is used on that page is referencing fields that no longer exist in the online form, and these fields are never submitted to JotForm. (They are never stored anywhere for that matter - such submitted data is lost forever.)
You should remove the source code version of that form from your second contact page, and instead embed an existing online form in its place. You can either create a new form, or you can edit and update your existing blank form 10881744157 here in JotForm with the fields you want it to collect.
Whichever the case, that new form will have to be embedded in place of the source code version of an obsolete form.
