Have lost username and password to account and need assistance in resetting

    I'm contacting you on behalf of my employer Great Expressions Dental Center. We have been using your Jot forms for a while now and we find ourselves in situation where our employee fimiliar and which created our form no longer with us. WE do not have access to the user name or password created and need to reset. Please contact me or Robert Ammons (reobert.ammons@greatexpressions.com) so we can get this resolved.
