How can i creat a payment with multiple options?

I want to include a master/visa card payment option into my form. I want it to have multiple price options, in which my clents cant select the prices they want.

Sample attached below. I just need something like that. Help please. I really want to upgrade my jotform account , but i cant until i know it can do what i want.

If you want to accept card payments, you may need to consider using PayPal Pro, Stripe or Authorize.net options.
