A few forms in Sept have not been emailed to me
They have been coming for months, but stopped,
I do not know why

One of your notifications is not properly setup:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar)
The email address was still in bounce list:
In addition to this, you may white-list our domain names, if the issue continues, as we use Amazon SES method for "email@example.com" sender.
