My forms will appear momentarily and then disappear! They were working fine over the weekend. I am Suzanne Brooks, and the form I am trying to load is "2016 Pledge Form."

The link that you have provided in the opening post is slightly different, with formjs in it, which is used in the script embed codes. This leads me to believe that you may be talking about this form being embedded somewhere, and that it stopped working there.
If your form is not loading on a page where it is embedded, Suzanne, could you please share with us the link to the page where it is embedded?

ANOTHER form ready and I'm going crazy. Actually, the form loads just fine
Just discovered something...I have been in Chrome (using a mac.) So, I
switched over to Safari just to see what happened, and now I'm in—just as I
was all weekend long!
I wonder what is happening with my Chrome platform?

It also loads fine on my Chrome browser at this time.
I assume all is good now on your form? Do let us know if there are still issue.
As to your Chrome platform issue, perhaps you just need to update Chrome browser?
