How do I embed my form on my webpage

I am trying to post the Puppy page form on my website.  How do I do this please? I want it to look uniformed people fill out and be able to submit to my email in outlook.

You can check our user guide on how to -Adding-a-Form-to-a-Web-Page

1. Click on the "Embed Form" button in the top toolbar

2. Click the "Embed" button

3. Copy the code provided

4. Paste the code into your web page where you would like to display it. Usually somewhere in the body section of your webpage and within tags
