form appears and then disappears immedietly

I am using avada theme on wp

I am embedding the code in my home page and the firm appears for 1 second and then goes away

right away. Here is the code I am using

I noticed that you are using script code for embedding form to your site. We recommend using of iFrame code. Please replace script code with iFrame code. You can see how to get iFrame code of your form in this article:
