We setup google spreadsheet integration for this form
We ran tests and it worked just fine. Now we have received submissions and the submissions are never making it to the spreadsheet? What could we do to remedy this? As stated we have changed nothing and it worked just fine when we tested during setup with zero issues.

Based on what you have mentioned, the integration between your form and Google Spreadsheet had been broken, so you would need to re-integrate.
Please do note that if you make any changes to the Google Spreadsheet sheet that is created by the integration itself, the integration will no longer work.
If you need to make any changes, my suggestion is to either, copy the data or import the data to a new sheet or spreadsheet.
You can also edit it through the sheet functions directly, but never manually.
To fix this, you should now:
1. remove the current integration
2. re-integrate again
