I need to migrate the email addresses from my forms to my Constant Contact account. I will need to create an Excel file to export them.

You can create an excel report for each one of your form and copy them from there:
You would need to create a report for each one of your forms, however, it does not look like you have too many forms so it should not take that long at all.
