We have an account that our computer guy set up and I am trying to upgrade to the 1000 a month teir. I do not have our username and password.

    The username of the account that created the form on that webpage is 'rendleman'

    The email address is the same as the one you used when making this post on the forum. 

    You should be able do a password reset for the account (at the bottom of www.jotform.com) with that information.
