How to include text on email notifications

I AM CREATING EMPLOYMENT APPLICATION FORMS, I RAN A TEST TO SEE THE REPORT I WOULD GET AND IT ONLY SENT ME A SIGNATURE AND NOT THE "TEXT"  ABOVE IT. WHAT DO I NEED TO DO FOR THE TEXT TO BE SENT WITH EACH SUBMISSION?

Text are not included on email notifications because they are static. However, you can modify your notification and manually add them there so they are included on every form submissions.
