My problem is similar and a bit more. I upload a document, hit save and continue, and later I access the form via the email with the session ID and want to replace my document. So I delete it. Hit Save & continue. When I access the form later via the session ID again, the document I deleted has appeared again.
Our applicants seem to be deleting/uploading new documents frequently in our grant application so upon final submission, we are actually receiving all their documents -- the ones that are uploaded as "final", as well as the ones they previously deleted.
I also tested this: I uploaded a file in a *required field, hit save & continue, went Back by pushing the back button, and deleted the file. I was able to save & continue again even though the required field was now empty. And when I went back again with the back button, the deleted file re-appeared. As up top, when I submitted this form, even though I deleted the file, it did come through in the submission.
Please let us know about deleting files with the Save & Continue feature.
I created a TEST version of our form (because all fields are required in the original form), the link of test version is below.

It is actually working for me, but the thing is that if you remove only the file, it seems to stick there. If you change some other detail on that same page and then remove the field and hit save it will no longer be there.
Of course, if should remove the uploaded file as it is being removed on the page, instead of leaving it if you come back and instead of requiring additional changes on the form, so I will raise this to our developers - I will forward your form to them to experience the same and see what might be going on in the back.
Also, thank you for linking to the old thread instead of posting a comment on it, this is the best way to do this and thank you for doing it like so :)

Thanks Ben, please keep us posted. I tried changing a field to check what you said, if the deleted file would stay removed, but it didn't work on my end. We might create two forms for a temporary fix, first form is text fields only, no uploads, and Thank You message populates unique session ID and links to a second form. Second form are the uploads only and requires applicant to enter their session ID from first form, so we can match the two forms upon submission. But if your developers can fix this soon, that would be super.

Actually I can not replicate the same steps that worked for me before, but luckily I can tell you that I can see one of our developers being assigned to this thread and that the bug has been marked as important, meaning that it is already being looked into this.
Now I can not give you any timeframe when it would be resolved, but as soon as it is, we will update you about this through this thread.
