Image is not showing in the form

Trying to add an image to a form but the image does not appear on the form. Tried using a cloned form and then a new form. Image is a .png file. Still no luck. Ideas? The image would appear in the box at the top of the form. Please see screen shot attached. Thanks

I checked the form and I can see the image properly. I checked the original form and the cloned form. Are you still experiencing the issue on your end? Can you please try another browser and see if it works. I will clear your form cache from our servers.
