    I am mapping my fields to Salesforce, and some of my form fields are not available on the pick list

    I can map all form fields to Salesforece fields, but the last two questions- How much would you like to borrow, and what were revenues in 2014 are not available as choices, even though I have fields for those labeled in Salesforce.

    To my understanding you mean that two fields on your form are not sending the information into your SalesForce Integration.

    Unfortunately, I'm unable to check this on your account since you have a guest account which we cannot log into unless you sign up for a free account which would allow us to check it.

    However, I made a copy of your form to test it which currently I found it appears that the reason this is happening is because those are two widgets being used instead of two fields which leads me to believe they are likely unsupported in Salesforce. 

    Additionally, to my current knowledge SF only supports textboxes and a few other fields with which it is a bit limited on. Please see https://www.jotform.com/help/321-Mastering-SalesForce-with-JotForm for a better understanding of how to set up and use JotForm with SF.
