It seems that the Thank you page was edited/changed from a custom URL which was not saved properly. I see that your form is now redirecting to a Thank you message. I've checked the Thank you page on your form and saved the Thank you message to redirect to it after clicking on Submit. It is now displaying the Thank you page.
