Upgrade My Account Difficulty

    I would like to upgrade this account and am willing to pay more, but my developer who created this jotform on www.mp45.com under contact did not leave me with the credentials to log into this after I fired him. Please get back to me so I can update this to the premium level. I receive emails through mp45@muscleprodigy.com but when I tried resetting the password there, that didn't work. Please get back to me soon. Thank you.

    First off, thank you for using JotForm. Let me please inform you that the form on your website was created as guest, inshort there's no account created for it. If you'd like to owned and managed that form, please create a new account at www.jotform.com and used this email on the registration form: mp45@muscleprodigy.com. After that, the form will be automatically transferred to your new created account.
