I created a question and I want to see it on the form

So I select the text option so I can enter a question.   I create a text box so the participant can enter an answer.  When I look to the inbox or see the email come in the question from the 'text' I entered is not on the form. I wish to see this question on the form when I print it out or view in the email.

I cloned your form to test it and all three of your Text fields are included in the notification:

If you wouldn't mind letting us know which field is missing from the notification, we will be happy to help you include it.  If you would like the label from the field included, rather than placing the text above it, add the text as the field label instead.

This will include the text while printing, but not in the email notification.
