    can I combine several checkbox questions into 1 question

    I have 5 questions with checkboxes that I would like to combine into one question. I would like the groups of the possible choices combined and separated by some white space and non selectible text. See below. Is this possible and if so how?

    The easiest way to accomplish this would be to use separate check box fields and style them to look as if they are one:


    However, the answers would be collected as if they were separate fields. 

    The labels are set to be aligned to the top of the field and the check boxes are indented to look like sub-categories. It is separate fields and will be treated as such.

    It would not be possible to have a single checkbox field that has text fields in between various options.
