    The form I created has a form width of 490. But when it is loaded to my website, the form background stretches the width of my website.

    I am interested in a narrow form that centers on the page, as it does on another page.  See http://www.cfar.emory.edu/vdc/register/oct.php.

     

    I set the form width to 490 for the form appearing on http://cfar.emory.edu/cores/ps/assistance_form.html but the form background extends across the entire width of the page.

    I took a look at your page and I see that the form is being given the width of 100% of the available space, instead of 490 pixels.

    To resolve this, please replace the js embed code with the iframe embed code. This will allow you to change the width from 100% to 490px instead.
